We want your knowledge and expertise! **Job Summary and Qualifications** Performs the clinical functions for the Department of Pharmacy. Will provide structure, guidance, and accountability from a clinical standpoint to customers (patients, physicians, employees, visitors, and investors) who receive pharmacy services in a professional and efficient manner. What you will do in this role: + Provide clinical pharmacy/pharmaceutical care for hospital patients. + Maintain current with clinical literature especially related to the hospital-specific patient populations. + Maintain competency to provide pharmaceutical care to all ages and types of patients that may be maintained within this hospital. + Serve as pharmacy representative for weekly team conference meetings; 5. Process automatic stop order forms; 6. Monitor patients on anticoagulant therapy, antibiotic therapy and make recommendations to physicians as needed. + Perform DUE as needed. + Serve as a member of P&T committee and present reports as needed. + Oversee ADR program; + provide drug information to hospital staff, administration and physicians. + Other Administrative Functions **What qualifications you will need:** + Currently licensed to practice Pharmacy in the State of Texas through the Texas State Board of Pharmacy and be in good standing with the Board of Pharmacy. + Experience in I.V. admixture and unit dose drug distribution system required + PharmD degree/ Doctorate Degree + Completion of a PGY2 in critical care (preferred) or equivalent critical care experience. + Board certified in critical care or pharmacotherapy preferred.
