Job Description
Position Summary
Under general supervision, this position will aid in clinical care coordination between the patient and various health care providers, professionals, and facilities. Reports to the Community Health Program Manager.
Duties/Responsibilities
- Examines, assesses, and screens individuals to coordinate initial, follow-up, and existing medical or other health services and appointments with various health care providers.
- Evaluates the residential living environment for safety, environmental, and nutritional considerations and risks.
- Assists with identifying primary healthcare, public health, mental health, behavioral and preventive wellness services for eligible citizens within the community.
- Works with the local Health Department, Community Services Board, and behavioral health facilities to promote and conduct patient education, prescription drug treatment, appointment retention, and lifestyle assessments.
- Provides eligible citizens with information and guidance related to diabetes prevention and treatment, hypertension, Congestive Heart Failure (CHF), Chronic Obstructive Pulmonary Disease (COPD), fall assessment, injury evaluation, geriatric frailty visits and self-neglect assessments, and nutrition.
- May assist in serving as the liaison to area hospital Emergency Services Directors, Operational Medical Directors, and to local, regional, and State Department of Health agencies, and EMS agencies and area behavioral health facilities.
- May work with local public health agencies, health agencies, health systems, and other providers.
- Prepares reports on issues involving services to include, direct contact and support, quality improvement, and quality assurance.
- Makes recommendations regarding expenses and purchases relevant to EMS and behavioral health training, operations and equipment, vehicles, and delivery of service to include grant funds.
- Performs other duties as assigned.
Qualifications
- 3-5 years of clinical, emergency medical treatment, or public health nursing experience, or an equivalent combination of education and experience.
- AHA Basic Life Support (BLS)
- AHA Advanced Cardiac Life Support (ACLS)
- AHA Pediatric Advanced Life Support (PALS)
- Geriatric Emergency Medical Services Training (GEMS)
- Community Paramedic Certification
- Florida Driver’s License
Education
- High school diploma or general education degree (GED)
- BS/BA or equivalent experience in related field preferred
- Paramedic or nursing certification required.
- Prefer associate or bachelor’s degree in Paramedicine, Nursing, Emergency Medical Services, Fire, or a related field.
Competencies
- Emergency Medical Services – Comprehensive knowledge of pre-hospital emergency medical procedures, techniques, methods, medication, equipment, and first aid. Thorough knowledge of Incident Management and supervisory practices relating to the emergency response principals and the provision of emergency medical care. Comprehensive knowledge of EMS and departmental standard operating procedures, practices, requirements, and regulations; knowledge of EMS service and applicable laws, liability, and legal issues.
- Safety/Security – Comprehensive knowledge of safety rules, regulations, procedures, and practices as outlined in the Newport News Fire department directives. Maintains situational awareness and ensures operational resource management.
- Customer Service – Comprehensive knowledge of principles and processes for providing customer services. This includes setting and meeting quality standards for services and evaluation of customer satisfaction. Ability to handle a variety of customer service issues with tact and diplomacy in a confidential manner.

Working Environment and Physical Requirements
- Requires the ability to exert light physical effort in sedentary to light work.
- Some combination of stooping, kneeling, crouching, and the lifting to include overhead lifting, carrying, pushing, and/or pulling of heavy objects and materials (up to 100 pounds) and occasionally heavier items (100 pounds or over).
