Job Summary

The Resource Navigator & Family Success Partner is a full-time position responsible for coordinating and delivering supportive services to families enrolled in Community Care Fellowship programs—primarily those participating in the Home Together Nashville initiative. This role blends onsite resource navigation for families in the CCF Family Shelter with ongoing case management functions as a Family Success Partner, helping families develop and implement individualized Family Stability Plans.The ideal candidate is aligned with CCF’s mission and values, demonstrates trauma-informed practice, and can build trusting relationships with families experiencing housing instability.

1. Family Shelter Support & Resource Navigation

  • Serve as the primary point of contact for the Family Shelter during shifts.
  • Respond appropriately to immediate family needs as they arise.
  • Answer incoming calls for the Family Shelter and direct families or callers to appropriate community resources.
  • Greet, coordinate, and support volunteers, including hospitality teams providing dinner and class facilitators.
  • Receive, track, and distribute family supplies; notify staff when restocking is needed.
  • Provide crisis management and de-escalation as needed.
  • Deliver exceptional customer service to families, volunteers, staff, and the community.
  • Maintain light housekeeping duties to ensure a welcoming shelter environment.
  • Adhere to all agency policies, procedures, and professional ethical standards.

2. Family Success Partner Responsibilities (Home Together Nashville Program)

  • Provide 20 hours per week of case-management-focused support specifically for families enrolled in the Home Together Nashville pilot.
  • Work with referred families to develop Family Stability Plans, ensuring goals reflect crisis stabilization, long-term stability, and healthy family growth.
  • Implement individualized, holistic support strategies in coordination with Home Together Nashville.
  • Participate in the development of processes and protocols to ensure consistent, high-quality wraparound services.
  • Maintain regular communication and collaboration with Home Together Nashville staff, including quarterly feedback and participation in program meetings.
  • Participate in the creation of a shared resource guide for the program and identify additional resources as needed.
  • Provide updates on family progress and coordinate with landlords or partners as appropriate.
  • Help monitor and evaluate the effectiveness of support services and suggest improvements.

3. Cross-Organizational Collaboration

  • Communicate regularly with both CCF leadership and the Home Together Nashville coordinator.
  • Participate in scheduled meetings, case reviews, and program development efforts.
  • Maintain accurate documentation and data in HMIS and other required platforms.
  • Uphold a strong commitment to trauma-informed care and family-centered service delivery.

Training Requirements

  • Housing Navigator Training (Office of Homeless Services) – Required
  • HMIS Coordinated Entry Training (OHS) – Required
  • Additional training as needed for professional growth.

Qualifications

 

Qualifications

Required:

  • High school diploma or equivalent.
  • Three years of professional social services experience (ideally with families).
  • Ability to handle confidential information appropriately.
  • Strong communication and organizational skills.
  • Basic computer proficiency.
  • Knowledge of community resources and social-service systems.
  • Ability to build rapport quickly with families, staff, and volunteers.
  • Commitment to trauma-informed care.

Preferred:

  • Bachelor’s degree in social work, human services, or related field.
  • Experience working with vulnerable or unstably housed populations.
  • Case management experience with a strong focus on workforce development.
