Palliative Medicine / Hospice NP (or PA)

Join an expanding Palliative Medicine & Hospice team in North Western Ohio (70 miles south of Toledo).  Support 6 patients/day.  Monday - Friday (8am-5pm).  Position requires 75% inpatient, including rounding for Hospice with 25% outpatient.  Assess, plan, implement, and evaluate care needs.  Symptom and chronic disease management, goals of care, and advanced care planning.

Requirements:

• Board Certified
• Palliative Medicine/Hospice experience desired
• Geriatrics experience required
• OH license, or eligible

Compensation:

• Guaranteed base salary (up to $130k)
• Productivity bonus (est $20k, no cap)
