Job Description

**Responsibilities** The Community Health Educator LVN possesses and maintains comprehensive knowledge about perinatal health, prediabetes, diabetes prevention, gestational diabetes, and management of Type 1 and 2 diabetes, Asthma, COPD, CHR and other chronic conditions. Develops and provides self-management education in the community and the hospital programs. Promotes and develops community engagement and education to promote equitable family engagement within the community. Will incorporate education principles that are current and reflective of evidence-based education and management. **Qualifications** + At least 2 years previous experience in patient education specific to chronic health conditions **REQUIRED.** + **Required** LVN; BLS - Must Possess and maintain a current Licensed Vocational Nurse license issued by the State of California and maintain a current Basic Life Support (BLS) Certificate. + Certified Diabetes Educator by the National Certification Board for Diabetes Educators **Preferred.** + Bilingual in English and Spanish **preferred.** **Overview** Founded in 1910, Dignity Health - Community Hospital of San Bernardino is a 347-bed, acute care, nonprofit, community hospital located in San Bernardino, California.
