St. Luke's Health System is seeking a dedicated and detail-oriented home delivery outpatient pharmacist Pharmacist to join our Home Delivery Department. This role is essential in ensuring the safe and effective delivery of medications to our patients in the comfort of their homes. The ideal candidate will have a strong clinical background, excellent communication skills, and a commitment to patient-centered care. **Key Responsibilities** + Review and verify medication orders for accuracy, completeness, and appropriateness prior to dispensing. + Provide medication counseling and education to patients and caregivers regarding proper medication use, potential side effects, and interactions. + Collaborate with healthcare providers to optimize patient medication regimens and address any concerns. + Oversee the preparation and packaging of medications for home delivery, ensuring compliance with safety standards and regulations. + Maintain accurate patient records and documentation in compliance with pharmacy laws and organizational policies. + Excellent communication and interpersonal skills, with a focus on patient education and support. + Ability to work collaboratively in a fast-paced, team-oriented environment. **Qualifications** + Degree from an accredited pharmacy program + Valid Idaho Pharmacist License **What's in it for you** At St. Luke's, caring for people in the communities we serve is our mission - and this includes our own SLHS team.
